Needs Assessment

At your assessment we will discuss your circumstances, your care needs and how they affect your ability to manage with daily tasks. We will look at what help you may already have in place and how urgently we may need to provide services, or whether we should refer you to other agencies.

If you have a carer, such as a friend or relative, who looks after you regularly and for a substantial number of hours each week, they will be offered an assessment too.

All information is treated as confidential, but we will ask you for your consent to share information with other people who may be involved in your care. This helps us to provide the best possible services.

What does the assessment tell you?
The assessment will tell us how you are managing in the following areas:

    • Personal Care (e.g washing, dressing) and daily routines
    • Eating and drinking
    • Getting about your home
    • Communication
    • Transport
    • Health and Safety
    • Household tasks (e.g preparing meals.)
    • Your independence and ability to make choices
    • Family involvement and wider community life

Can I have someone with me at my assessment?
Yes you can, and this can be any person of your choice, e.g. a relative, friend, carer or advocate. If you do not know anyone who can be with you at your assessment, but you would like support from and independent person, we will try to help you find someone.

Eligibility
Because we need to make sure that we provide services fairly and to those in need, we use eligibility criteria to help us make decisions about who we can provide services to.

If you are eligible for Council Services, we will arrange with you a Care Plan. We will also undertake a financial assessment, to determine whether you will need to pay a contribution towards the cost of your care, and how much you may need to pay.

Required Customer Information
Date of Birth and confirmation of address will be required.

Service Level Agreement and Turnaround

Our aim is to ensure that we contact you within 48 hours of a new referral. Following an assessment, if you are requiring a service and meet our eligibility criteria, we aim to provide a service within 28 days.
