Advice and support for those with a physical, sensory or learning disability, who may wish to seek full-time or part time employment and who having been assessed under the NHS and Community Care Act are considered as meeting the Council’s eligibility criteria. The service centres will provide advice on skills training, liase with other agencies eg OT, Disability Employment Advisor and support to find appropriate work experience, volunteering or employment opportunities.

The LD & PD service centre staff provide advice to people with a physical, sensory and/or learning disability as a part of the care management process and the promoting independence agenda. This will usually be following an assessment and determination of eligibility for services including on going support around employment issues.

Any one can self refer (or be referred) to the PD, SI and HIV Team or the Learning Disabilities Team if they have a learning disability, physical disability, long term health condition, visual impairment, hearing impairment or HIV/AIDS. On making contact with the duty service (details above) a duty social worker will request detailed information to enable the referral to be prioritised. A community care assessment will be carried out which may involve other people/agencies (with your permission). The assessment will highlight areas of community care need and eligibility for service. Following this, if eligible a care plan will be devised to help a person be as independent as possible and to assist with areas of difficulty relating to their disability. A care plan, may include personal care, community activities, training or education or access to employment. The care plan will be reviewed at least annually to ensure that the services provided continue to meet your needs.
