Home Care

If you need assistance with personal care, such as washing or dressing, or if you need help with eating and drinking, and other activities of daily living, you may be offered home care services.

Referrals are made from the Older People’s Assessment and Care Management Team, or from a Health professional, to the Purchasing Monitoring and Review (PMR) Team. The PMR Team operates a daily response service (duty system) for clients and service providers

The PMR Team is responsible for monitoring home care services for people aged 65 plus. They undertake timely reviews of all service users, recommending any necessary changes to existing care services provided.

Eligibility
Services are arranged following a Needs Assessment. Because we have to make sure that we provide services fairly and to those most in need, we use eligibility criteria to help us make decisions about who we can provide services to.

If your Needs Assessment shows that you are eligible for Council Social Care services, we will carry out a Financial Assessment to determine how much you may need to pay for your services. You will also be offered Direct Payments so you can arrange services yourself instead of the Council arranging them for you.

Required Customer Information
Date of birth, proof of address and identification documents are required prior to needs assessment.

Service Level Agreement and Turnaround
Following receipt of Needs Assessment and Care Plan, services are normally arranged within three days. In an emergency situation, home care can be arranged within 24 hours following a Needs Assessment.

An Out of Hours Service operates Saturdays, Sundays and Bank Holidays, 11.30 am – 3.00 pm. In an emergency situation out of hours, please telephone the Customer Service Centre on 020 8379 1000 and ask to speak to the Emergency Duty Social Worker.
