Needs Assessment | |
At your assessment we will discuss your circumstances, your care needs and how they affect your ability to manage with daily tasks. We will look at what help you may already have in place and how urgently we may need to provide services, or whether we should refer you to other agencies. If you have a carer, such as a friend or relative, who looks after you regularly and for a substantial number of hours each week, they will be offered an assessment too. All information is treated as confidential, but we will ask you for your consent to share information with other people who may be involved in your care. This helps us to provide the best possible services. What does the assessment tell you?
• Personal Care (e.g washing, dressing) and daily routines
