Physical Disabilities, Sensory Impairment and HIV Team
This service provides a confidential service, counselling and emotional support, financial and practical advice to people diagnosed with HIV. We provide a support group for service users and their families called Umoja for people living with HIV.
On referral, a comprehensive assessment is triggered by the presence of a permanent or substantial disability, long term illness or condition. This is undertaken by a social worker or Rehabilitation Officer. An Assessment and Care Plan is completed, identifying needs and services to be provided to meet these individual needs.
A weekly support drop-in service is provided for African and Caribbean people and their families living with HIV. This is user-led, and provides the following support:
• Regular seminars and health talks on treatment & medication.
• Workshops on immigration and Asylum Issues.
• Creche facilities provided for children under 8 years.
• Peer support.
• Support to families.
• Advice.
• Information.
• Practical support.
• Signposting.
Eligibility
The service is for adults diagnosed with HIV between the ages of 18 and 65 years, and who meet the Fair Access to Care Services (FACS) elegibility criteria.
Required Customer Information
We require proof of diagnosis and details of treatment centre prior to start of Assessment and Care Planning process.
Service Level Agreement and Turnaround
We will contact the referrer within two days of receiving the referral for services. The Assessment process and provision of services will be completed in 28 days.
